The purpose of this ancillary study is to evaluate the effect of home blood pressure monitoring with nurse-directed telemedicine on the diagnosis and treatment of severe postpartum hypertension amongst women with known chronic hypertension compared to standard of care clinic visits 7-10 days postpartum.
