Neighborhood Health Partnerships Program logoThe Neighborhood Health Partnerships (NHP) program is dedicated to advancing community health. Our primary aim is to provide local, timely, and actionable health data and tools that empower communities, government entities, researchers, and more.

Wide health differences exist between neighborhoods, yet local-level health data is often not readily available. This gap makes it challenging for community organizations, health systems, government agencies, researchers, and others to identify and prioritize local improvement opportunities or target neighborhoods facing inequity.

Our data, tools, and examples are available to:

  • Identify new or confirm existing opportunities for improvement
  • Inform strategies
  • Strengthen proposals to secure resources
  • Build supportive partnerships
  • Advocate for change
  • Support evaluation in Wisconsin neighborhoods
