Overview
The Neighborhood Health Partnerships Program produces reports that present electronic health record data from Wisconsin health systems at the ZIP code level on 27 measures spanning the care continuum – from vaccinations and screenings to chronic disease care.
These reports include electronic health record data for any individual who received primary care from a participating Wisconsin Collaborative for Healthcare Quality (WCHQ) member health system. The reports also use U.S. Census American Community Survey (ACS) data and CDC Behavioral Risk Factor Survey (BRFS) data.
